The original document from which these scans were made was the Alphabetical Index to Official Roster of the Soldiers of the State of Ohio in the War of the Rebellion that was edited and compiled by the Works Progress Administration of Ohio in 1938 The index was created as part of official project 465-42-3-467 in District #4 of Ohio and was sponsored by the Office of the Adjutant General of the State of Ohio.

These scans were made by the team at the Soldiers' and Sailors' Monument in Cleveland, Ohio from 2019-2020. They were created from microfilm copy of the original index that varied in quality, and were uploaded to the Internet Archive by Cleveland Public Library.
